Collaborative Health Consulting

An initiative that leverages various community strengths to improve health outcomes. Together we create a more well rounded model of health and wellness for low income patients struggling with chronic disease management. In turn, communities as a whole become healthier and more resilient.

A Community Collaborative Healthcare Initiative

Thank you to our grant sponsors DPHHS for a grant called “Food Farmacy” that got this whole project rolling and Share Our Strength “No Kid Hungry”~

Both grants helped to more fully support this cohort of 10 food-insecure patients struggling with chronic disease management.

Thank you!

Patients living with chronic disease in underserved communities face significant challenges to eating healthy. To support them and improve health outcomes I believe, we need to integrate more community resources into the health care setting. The “prescription” for whole food or “Food Rx” is a promising platform for that community-linked intervention since having access to quality food is a vital component of health. Our partners span from university faculty and grad students to yoga teachers and farmers.
